What the position looks like:

Self- directed, independent, and autonomous work in case management involving one-on-one patient care in the comfort of where they call home. Develop deeper relationships and work in a more personal setting than a traditional medical office with the flexibility to build your own full-time schedule.

Work at the top of your license in areas such as wound care, IV/ infusions, cardiopulmonary assessment, diabetic teaching, post-surgical care, disease management, medication reconciliation and management, gastrostomy tube care, injections, blood draws, in-dwelling catheter care and management, and more. Assess patient needs and develop an individualized plan of care with the physician. Coordinate and oversee the care plan with the interdisciplinary team as the designated case manager. Patient demographics are elderly, critically ill, disabled, may be recovering from a surgery, injury, or accident, or a combination. Uphold quality improvement, safety, and infection control initiatives.

What you will need:

· Current active RN license in good standing with the state of Idaho

· CPR certification, we will provide if needed

· Demonstrate knowledge, skills, and abilities in self-directed, independent, and autonomous work

· Positive attitude for learning and growing
